Tragic deaths in custody point to a system in need of reform

Five young people in custody died in the space of just 33 days during March and April. The tragedies mark an exceptionally horrific spell in the youth prison system. To put it in context, no more than five teenagers have died in custody during any entire year since 2005, when the figure was nine.

On the face of it, the education select committee's call to split Ofsted into two separate inspectorates for education and children's care would represent a further step away from services centred on the needs of the whole child. It is a trend played out in several areas through the disappearance of children's trust arrangements and local authority children's services departments.

There was once a rather intimidating youth service inspector whose opening question was invariably: "What do you achieve?" Some colleagues, in anticipation of his visit and this question, constructed elaborate unit plans setting out their intended "outcomes", while others blagged an answer on the spot.
